Below are links to enrollment forms for the 2018-19 school year.  If you will be paying tuition, see the Tuition section below and be sure to complete the Tuition Payment Agreement and follow instructions carefully.  If you will be applying to the State of Wisconsin Choice Program, see that section below.

Racine Parental Choice Program (RPCP)

PSCP Disclosure of Information

The Racine Parental Choice Program enrollment for the 18-19 school year begins February 1, 2018. The DPI link to apply is https://dpi.wi.gov/sms/choice-programs/student-applications.  All families are encouraged to complete the application before the February deadline.  When asked to choose a school, click on "Siena Catholic Schools of Racine".

Income and residency documents may be dropped off at St. Joseph's or St. Catherine's (1200 Park Ave, Racine) or scanned and emailed to admissions@sienacatholicschools.org.  Be sure this is completed during the time frame you apply.

Immunization Information

Wisconsin state law requires all public and private school students to present written evidence of immunization against certain diseases within 30 school days of admission.  The current age/grade specific requirements are available from schools and local health departments.  These requirements can only be waived if a properly signed health, religious or personal conviction waiver is filed with the school.

For Step 2, you may manually complete, ask your physician for a copy OR go to the WIR (Wisconsin Immunization Registry) to print out your child's record to attach to the form.  You'll need your child's DOB and social security number to access their record.
